Formulate Pressure Ulcers Prevention Reposition Strategy for by the Distribution of Pressure Capture

  ABSTRACT :

 

Pressure Ulcers are caused by overload pressure on tissue from contacting between skin and bed surfaces; when outer pressure is bigger than inner pressure, it makes blood vessel stocked, which causes tissue damage (necrosis). If patients have Pressure Ulcers that means it will cost more medical resources including extra medical personnel, nurses and the mental load of patients themselves. In America, there are about one million patients with Pressure Ulcers per year. In average, they have to pay extra 2,000 to 70,000 dollars to hospital because of Pressure Ulcer.

Through healthcare personnel observations in 宜蘭榮民總醫院 圓山分院 and references of bedridden patients position, this research summarizes 4 well-stated positions: supine, 30 degrees tilt, lateral (Upper leg is positioned in front of the patient 上方腳在前), lateral (Upper leg is positioned behind the patient 上方腳在後). This research analyses between pressure data of bed surface by Tactilus® Real-Time Surface Pressure Mapping Technology and the pressure point distribution from references to conclude following points: (1) 30 degrees tilt position makes patients spread the load on the bed and reduce that pressure concentrates upon contacting points. Comparing with supine position, there is no clear pressure point at shoulder and bottom bed contacting area. (2) Leg cross strategy of lateral position lower the pressure between knees. Beside, pressure concentrated situation is reduced in Trochanter major (股骨大轉子) even in humerus (肱骨). Actually, Leg cross strategy is efficient to spread pressure concentration in lateral position that change the negative appraisals lateral position. (3) Assistive pillows decrease the pressure concentration and help to fix positions. (4) Weight is a significant factor to cause Pressure Ulcer but height is not. (5) It is an important factor of the order in change positions sequence strategy that the pressure concentrated areas overlaps between lateral position and 30 degrees tilt position. Finally, after, evaluated these factors of pressure concentration and positions. This research provides multi-positions changing strategy.
